Cell-Assisted Lipotransfer (CAL)
The CAL breast enhancement procedure generally takes about four hours, consisting of the following steps:- Anesthesia
- Liposuction
- Cell processing
- Enhancement (lipoinjection)
Step 1: General and Local Anesthesia
General anesthesia is used for both the liposuction and lipoinjection procedures. Clients are anesthetized for about four hours on the CAL procedure day. The liposuction and lipoinjection are both completed at this time. Fluid containing saline and epinephrine will also be injected into the areas where fat is to be removed.
This process, along with preparation for the rest of the procedure, takes less than 30 minutes.
Step 2: Liposuction
Liquid (composed of saline and epinephrine) is injected into subcutaneous fat. The epinephrine constricts the blood vessels to control and limit any internal bleeding. The liquid solution itself assists in the easy insertion of the cannula that is used to aspirate the fat. A very small incision is made in the skin and a narrow cannula, connected to a liposuction pump, is used to remove subcutaneous fat. More than one incision may be made in order to acquire enough material for the enhancement procedure. During the liposuction phase, some patients may request that additional excess fat is removed – beyond that which would not be needed for the enhancement procedure.
This step takes about 45 minutes.
Step 3: Cell Processing

Half of the fat that was removed during liposuction has the adipose stem cells extracted from it. In addition, stem cells are extracted from the liquid portion of the liposuction product. Finally, in preparation for the next step (lipoinjection), the stem cells are mixed with the fat that is to be injected into the breast.
This step of the process takes about 90 minutes.
Step 4: Enhancement (Lipoinjection)
The fat enriched with stem cells is injected using a patented applicator, which distributes the mixture evenly throughout the breast preventing the possibility of lumps.
This step of the procedure takes about 1 hour.
